To ensure we have all of a patient’s relevant medical information at their visit, as well as eliminate unnecessary visits, we request a referral from a referring provider or hospital emergency room.
Not all patients who have a neurological problem are surgical candidates. Dr. Alsina and his physician assistants personally review all incoming referrals to evaluate the severity of a patient’s condition and to determine if a neurosurgical evaluation is the appropriate course of action before accepting a patient.
We hope this does not inconvenience you; we are simply trying to provide the best medical care possible and avoid wasting the patient’s time and money.
Once the referral is received by our office, we will call the patient to schedule an appointment with either the doctor or his PA as quickly as possible. We will also notify you of the appointment information and copy you on the note as soon as it’s available.
An updated copy of our referral form is located here. Please make sure you send all relevant medical records, radiology reports, copies of insurance cards, and any prior authorizations or geographical gap authorizations as required by various insurance companies. Email the referral form to firstname.lastname@example.org or fax to (910) 341-7911.
